No — questions test only what the uploaded text actually says, and every question cites the page or section it was drawn from so you can verify it in one lookup. Still, review the bank before you grade with it.
A mix of multiple choice (four options, one correct), short answer with a model answer, and true/false — roughly half multiple choice by default. Your instructions can change the mix, the difficulty spread, or the language.
Yes — alongside the XLSX you get a matching CSV with the same columns, which most LMS and quiz platforms can map on import, plus a printable DOCX quiz with the answer key at the end.
